Health Care Professional Corporation Applications

A nurse may form a health care professional corporation with another health professional (e.g. physician) for the purpose of offering two or more kinds of professional services, pursuant to SDCL 47-11 by applying to the Board for a certificate of registration, pursuant to ARSD 20:48:14. 

• New applicants must complete the Initial Application and submit to the Board with the required documentation and fee.

• Nurses who hold a health care professional certificate must renew annually by November 1st. Submit the Renewal Application and fee, applications will be accepted beginning September 15th.
