  Reinstatement of Qualified CNAs

• An expired or inactive CNA who meets required training and employment hours may reinstate to an active CNA status
• Print and submit the Lapsed Reinstatement Application  to the Board of Nursing.

 Competency Evaluation Program Requirement

A CNA who does not meet the required renewal requirements for training and employment hours, must pass a Competency Evaluation Program, consisting of the CNA Knowledge Exam and a Skills Evaluation. See Initital CNA Registration. 

• Submit the CNA Reinstatement Application: Competency Evaluation Program Required to the Board of Nursing.
