Submit a Complaint

On a licensed nurse:

  1. Complete and submit the Complaint Form
    • Or, send an email with responses to the questions asked on the form to the SDBON Investigator
  2. After submission, the Board will contact you to acknowledge receipt 
    • Please be aware, the investigative process and final outcome may take several weeks or months to complete. As the complainant, you will be notified of final actions taken by the Board.

On a registrant (CNA and Medication Aide)

  1. Complete and submit the Complaint Form
    • Or, send an email with responses to the questions asked on the form to the SDBON Investigator
  2. After submission, the Board will contact you to acknowledge receipt
