Submit a Complaint

Any person claiming that a licensed nurse, registered CNA, registered medication aide, or an applicant, has engaged in or is engaging in conduct that may be grounds for disciplinary action, may file a written complaint with the Board.

To make a formal complaint, you must submit the Board's required complaint form. You will be required to provide the full name of the nurse, registrant, or applicant and a detailed description of the alleged conduct that you believe violates the Nurse Practice Act: SDCL 36-9-49 or ARSD 20:48:16. 

• Failing to submit the Board's complaint form and necessary documentation will be a basis for the Board to reject the complaint without further action.
• The Board's Compliance Director may contact you for additional information or documentation.  

Complaint on a Licensed Nurse or Applicant

• Complete and submit the online form to issue a complaint on an LPN, RN, CNM, CNP, CRNA, or CNS: Complaint Form

Complaint on a Registered CNA, Medication Aide, or Applicant

• Complete and submit the Complaint Form to issue a complaint on a CNA or registered medication aide.
• After submission, the Board will contact you to acknowledge receipt
