Submit a Complaint
Any person claiming that a licensed nurse, registered CNA, registered medication aide, or an applicant, has engaged in or is engaging in conduct that may be grounds for disciplinary action, may file a written complaint with the Board.
To make a formal complaint, you must submit the Board's required complaint form. You will be required to provide the full name of the nurse, registrant, or applicant and a detailed description of the alleged conduct that you believe violates the Nurse Practice Act: SDCL 36-9-49 or ARSD 20:48:16.
- Failing to submit the Board's complaint form and necessary documentation will be a basis for the Board to reject the complaint without further action.
- The Board's Compliance Director may contact you for additional information or documentation.
Complaint on a Licensed Nurse or Applicant
- Complete and submit the online form to issue a complaint on an LPN, RN, CNM, CNP, CRNA, or CNS: Complaint Form
Complaint on a Registered CNA, Medication Aide, or Applicant
- Complete and submit the Complaint Form to issue a complaint on a CNA or registered medication aide.
- After submission, the Board will contact you to acknowledge receipt
Please be aware, the investigative process and final outcome may take several weeks or months to complete. As the complainant, you will be notified of final actions taken by the Board.
