Disciplinary Actions on Nurses

• Primary source verification of a nurse's license and public disciplinary documents pertaining to actions taken by the Board against a licensed nurse, LPN, RN, CNM, CNP, CRNA, or CNS, may be accessed and printed from the Board's License Verification Portal .

• Disciplinary actions reported by the Board are also listed in this Disciplinary Actions Document.    

• National nurse licensure and disciplinary information may be accessed in the National Nurse Licensure Database.
