Certified Nurse Aide and Registered Medication Aide

• Login and change your address or update your contact information using the online Address Change form. 

• Complete and email the Name Change Request Form along with a copy of the legal document that makes your name change effective, such as a marriage license or divorce decree, to the Board of Nursing's office. Upon receipt, your name will be changed on your registration record. 

• Until your registration record is updated on the Board's Verfication Site, continue to use your former name in relation to your practice as a CNA or Registered Medication Aide (RMA).
