Skills Evaluation Information
CNA Required Skills Evaluation
- To meet the CNA Skills Evaluation requirement, the Board developed a pool of 22 Skills Checklist Forms.
- A student is randomly assigned 4 of these skills that an RN Skills Evaluator must use to evaluate the student. The skills are randomly assigned by the Board based on the month and day of the student's birth.
- RN Skills Evaluator Qualifications: the Program Director is responsible to appoint the RN Skills Evaluator who must be an actively licensed RN with a minimum of one year of licensed nursing experience caring for older adults or the chronically ill of any age. The DON in the nursing facility is not eligible.
- The forms are similar to the skills checklists found in Hartman’s Nurse Assistant Care: Long-Term Care, 5th ed. Workbook.
- The Board-approved Program Director will be provided access to print the forms, see below, and must provide the forms to the RN Skills Evaluator(s).
- The RN Skills Evaluator may evaluate the student during the NATP skills lab or at the end of the 75-hour NATP.
- After the forms are completed, the Program Director must email all 4 of the forms along with the student's NATP Completion Verification Form to the BON at SDUAP@state.sd.us.
- If the Program Director delegates the task of emailing the forms to another individual, the Program Director must also be included / cc'd on the email.

Program Director Access to Print Skills Checklist Forms

A request from a Program Director to give access to another individual will not be approved.
-
After submitting an email request, allow 5 business days and the Board will send an email with sign-in instructions to the Program Director.
