Skills Evaluation
Skills Checklist Forms
To meet the CNA Skills Evaluation requirement, the Board developed a pool of twenty-two Skills Checklist Forms. A student is randomly assigned four of these skills based on the month and day of the student's birth. An RN Skills Evaluator will use these four checklist forms to evaluate the student.

The Skills Checklist Forms are similar to the skills checklists in Hartman’s Nurse Assistant Care: Long-Term Care, 5th ed. Workbook.

The RN Skills Evaluator may evaluate the student during the NATP skills lab or at the end of the 75-hour NATP.

The Board will only provide access to the Skills Checklist Forms to a Board-approved Program Director. The Program Director is responsible for giving the forms to an RN Skills Evaluator.

After the RN Skills Evaluator evaluates a student on all four skills, the Skills Evaluator gives the completed forms to the Program Director to email to the Board at SDUAP@state.sd.us.

If the Program Director has another individual email the forms to the Board, the Program Director must also be cc'd on the email to the Board.
Access to Forms

A Board-approved Program Director must request to access the forms by Emailing the Board at SDUAP@state.sd.us.

Be advised, a request from a Program Director to give access to another individual will not be approved.

After submitting an email request, allow 5 business days and the Board will send an email with sign-in instructions to the Program Director.
Checklist forms may be printed from the Skills Evaluation Checklist portal.
