Submit a Complaint

Any person claiming that a registered nurse (RN), licensed practical nurse (LPN), certified nurse midwife (CNM), certified nurse practitioner (CNP), certified registered nurse anesthetist (CRNA), clinical nurse specialist (CNS), certified nurse aid (CNA), or a registered medication aide (RMA), or an applicant, has or is engaging in conduct that may be grounds for disciplinary action, may file a written complaint with the Board.

To make a formal complaint, you must submit the Board's required complaint form.

You will be required to provide the full name of the nurse, registrant, or applicant and a detailed description of the alleged conduct that you believe violates one of the grounds listed in SDCL 36-9-49 or ARSD 20:48:16

Failing to submit the Board's complaint form and necessary documentation will be a basis for the Board to reject the complaint without further action.The Board's Compliance Director may contact you for additional information or documentation.  After submission, you will receive an email from the Board confirming receipt.

Be advised, the investigative process and final outcome take several weeks or months to complete. As the complainant, you will be notified of the Board's final action.
